Chronic Care Plus of Polk County
ABC for Rural Health convened a coalition of the three critical access hospitals in Polk County (Amery Regional Medical Center, Osceola Medical Center, St. Croix Regional Medical Center) to open a “virtual” free clinic. In contrast to the typical brick and mortar clinic, each hospital agreed to set aside a certain number of patient slots for uninsured, low income adults with chronic health problems. During the first year of operation, 60 patients were referred for treatment, with a goal of 200 in the future. This project was made possible by a 2011-12 Rural Community Grant from the Wisconsin Office of Rural Health.
