Rural Health Initiative Expansion Pilot
Rural Health Initiative partnered with New London Medical Center and the Outagamie Health Department to expand their successful “kitchen table health care” program. The program is designed to serve farm families who, even if they have health insurance, are often unable or unwilling to travel to see a physician when needed. Chronic health problems may go undetected or untreated. RHI worked with a county public health nurse who visited the farmer’s homes and conducted basic health assessments, physician referrals as needed, and provided health education. This project was made possible by a 2011-12 Rural Community Grant from the Wisconsin Office of Rural Health.
